To whoever it may concern,

I wish to formally express my concern regarding the possible unauthorized use of my personal identity. It has come to my attention that unknown parties may have taken out life or health insurance policies in my name, without my knowledge or consent. Such actions could amount to wagering on my health condition or even my death, with financial gain being sought at my expense. I respectfully request clarification and assurance that no such policies exist under my credentials, and that my identity remains fully protected against any misuse.
